CONDUIT™ - Quality Use of Medicines (QUM)

The QUM Project is embedded within Phase 3 of the CONDUIT™ Program. It aims to examine whether sharing of electronic patient information between hospital health care providers (diabetologists, hospital pharmacists) and community health care providers (GPs, community pharmacists) improves QUM (focusing on 2 indicators: medication prescribing and medication review) for patients with type II diabetes.

It investigates the feasibility and viability of linking electronic patient data between hospitals, general practices and community pharmacies, and will develop a diabetes management intervention based around the sharing of electronic information. It will also investigate the impact of electronic data sharing on the users and adherence to diabetes management, hypertension control and lipid management guidelines.

Following an initial phase of literature review, fact-finding and exploration of barriers and facilitators to data linkage between health professionals and the impact such linkages have on chronic disease management, in particular diabetes management, and QUM, the project aims to apply CONDUIT™ at different professional sites in a contained environment. Clinical management and outcome indicators will be evaluated against key recommendations by the National Prescribing Services.
