Our vision

The vision for the CONDUIT™ is to support and deliver an integrated approach to the management of chronic disease by inter-professional teams to improve the health and well being of patients through quality, collaborative, multi-disciplinary, culturally sensitive health delivery, education and research.

Our mission

CONDUIT™ aims to establish standards and protocols to link existing databases in hospitals, general practices and allied health practices within the secondary and primary health sectors to enable the sharing of information to support clinical care and monitor the risk factors and health of patients. CONDUIT™ also aims to establish, partner and collaborate with research and evaluation projects to facilitate health research and evaluations.

Funding

CONDUIT™ is funded by the University of Melbourne as well as the Commonwealth Department of Health and Ageing PHCRED and UDRH programs via the Department of General Practice and School of Rural Health.
